Listed below are the items, both paperwork and clerical,
that you are required to turn in or take
care of at your respective State Meet check-in:

1) Properly filled out meet waivers, completed in ink,
with all requisite information and signatures.
2) Updated Health Insurance Information Forms –
if applicable for any of your team members
3) LHSPLA Equipment Waiver
4) Entry Fees - $20.00 per lifter
5) All LHSPLA fines paid/cleared – if applicable
