  The link below contains a questionnaire that all LHSPLA certified officials are asked to fill out and return to the Association's P.O. box. The two forms contained a) a form for you to update information such as change of address, e-mail, etc. and b) a form that you need to return concerning the meets that you will be available to work at this year. Please take the time to fill these out or e-mail the information to Chris Ledoux
